In order
for you to be licensed as a Pharmacist in the United States,
you need to successfully pass the NAPLEX (North American
Pharmacist Licensure Exam) and the MPJE (Multistate Pharmacy
Jurisprudence Exam). Information about the exams is available at the National Association of Boards of Pharmacy website http://www.nabp.net.

the Board no longer provides copies of this booklet to give to
students. However, you should review the document very carefully. Students
should also exam the link for the blueprint of each exam. (Click on the
Applications link on the right of the screen, select NAPLEX from
the drop-down list, and then review the new links on the left hand side
of the screen. Generally speaking, questions related
to licensure should be directed to the Board of Pharmacy in the State
for which you are applying for licensure. Links to each Board of
Pharmacy can be found by visiting the National Association of
Board of Pharmacy website, http://www.nabp.net and clicking on "Boards
of Pharmacy" link on the right side. In addition to registering for
the exams as described above you will need to fill out an application
for the state you are applying for licensure in. For graduates applying for licensure of New Jersey

The Board of Pharmacy provides the instructions for
applying for licensure, and for the NAPLEX and MPJS exams, and information on
the fees involved. The forms are contained under the link titled, Application Packet for Licensure by Examination
or Score Transfer
The instructions from the Board indicate that you will need
to have a criminal history background check. This is in addition to the
background check you took prior to starting your advanced practice rotations.
The fee for this is approximately $33.00. The application fee is $125.00. The Actual
licensing fee is $70.00 for one year or $140.00 for 2 years. (Whether it will be $70 or $140
depends on when you submit the application.)
The instructions also explain that it is a good idea (we
agree) to take the pre-NAPLEX exam, for practice. Information about this, and much
other useful information is available in the National Association of Boards of
Pharmacy website: www.NABP.net (there is also a fee for the pre-NAPLEX).
You must submit to the Board an
official copy of your transcript indicating that you have graduated with a
Pharm.D. degree. An official transcript can only be obtained from the University
Registrar (not from the Pharmacy School's Dean’s Office). You can order the official transcript
online from the Registrar at : http://registrar.rutgers.edu/NB/TRANS-NB.HTM#official
The
version you submit to the Board must have your degree information
posted on it. When completing the request if you are asked about
waiting for degree posting indicate yes. Because the transcript must indicate that you have
graduated, you must wait until the date of the University-wide commencement (May 21st) to
order the transcript. You should check with the Registrar to be certain that your
graduation appears on the transcript. If your online transcript indicates the degree
(at the bottom after Spring 2008 grades) it will also be on the printed transcript.
It’s possible it will appear a few days prior to the University Commencement.
If Cycle 8 is your final rotation: Once you have successfully completed all of the rotations,
you may apply to take the NAPLEX and MPJE exams, and can even take the exams,
but your license will not be issued by the Board until your official transcript
has been submitted. If you can arrange to take the two exams before you
graduate, this will obviously speed up the process, and you will obtain your
license that much sooner.
For those
students who will be finished with Cycle 8 to
apply to take the exams before you graduate, there is a form (the last
page of
the online forms titled, “Certification of College Record of Applicant
for
Examination,”) that must be filled out, signed, and sealed by the Dean.
You can fill out this form and drop it off in the Dean's Office to be
signed. We will call you when the Dean has signed it. However, you
must include a printed copy of your unofficial transcript (from the
online system) and MUST INCLUDE a written statement indicating that
you have successfully completed all of your rotations. (i.e. they have
received
all your Spring grades) from the Department of Pharmacy Practice by contacting Marta Kaminska at 732-44-5215
x417. Thus for students completing the program with Cycle 8, you would drop off to thte Dean's Office: - Printed copy of your online transcript.
- Written Statement from Pharmacy Practice regarding Spring 2008 grades have been received.
- Certification of College Record of Applicant for Examination.
Written statements will not be available until March 24, to allow us
to receive Cycle 8 grades and grade your portfolios.
If Cycle 9 is your last rotation: Students who are
taking Cycle 9 DO NOT need to have this form completed and can simply use the
transcript as verification. If you are on rotation during Cycle 9 do not request the Written Statement from Pharmacy Practice.
While we will be happy to try to answer any questions you should
have, but please remember that we at the school are not the licensing
agency, so official answers can only be obtained from the Board. We are acting in an unofficial
advisory capacity only to try to be helpful. I suggest you download and read the
application packet before you do anything else. One final bit of advice: When filling out
the forms, use “Rutgers, the State University of New Jersey” as the name
of the School from which you are graduating with the Pharm.D. degree.
For all other states, it is
the responsibility of the student to obtain the forms from the
Board of Pharmacy in that state where he or she intends to

Notary Public Information Specific
rules need to followed regarding the notarization of
documents, especially one which more than one person is
signing. First and foremost, all parties signing the
document and the document itself must be present in front of
the Notary. Secondly, each signee must provide the Notary
with two forms of ID that contain the individual's signature.
At least one of the IDs needs to have a relatively recent
photo of the bearer. Valid IDs include, but are not limited
to, valid passports and driver's licenses. A Notary at the
University may also accept your current Rutgers University ID
at his or her discretion. Third, a document may
not be signed prior to visiting the
Notary. It must be signed in front of the Notary.
Notary
Publics are available at banks, law offices, pharmacies, and
most copy stores. Be sure to make an appointment with the
Notary to insure that the person is available. Make sure that
all parties who will be signing the document will be able to
be available for the appointment. Ms. Urbanski in the Office of Academic Services is a notary public.
Depending
upon the document and the Notary, a small fee for the service
may be charged. In the State of New Jersey, the fee may not be
more than $2.50. Most Notary appointments will last ten to
fifteen minutes.
